Schedule a Healthcare Assessment Test

Certified Nursing Assistant (CNA) / Phlebotomy

Eligibility Requirements and Registration Instructions

Students must provide proof of one of the following eligibility requirements to register for CNA or Phlebotomy. (Registration is first come, first served) If unable to meet eligibility requirements please proceed to the Health Care Assessment Testing Procedures section.


Program Eligibility:

Eligibility One

·         Eligibility for ESL 152 or eligibility for ENG 084 and ENG 074 or above and

·         Eligibility for MAT 075 through a transcript review

·         An academic record that meets the criteria listed above is verified through a WES transcript.

Eligibility Two

·         Meet either College Credit Exemptions or SAT or ACT exemptions according to the guidelines listed on the other side.

Eligibility Three

·         Achieve or exceed the following Accuplacer scores through the NCC Placement or Health Care Assessment tests

o   30 or over in Arithmetic

o   English Scores

§  Reading Score 51 or above and

§  Sentence Skills 67 or above

or

§  Students with Essay results of 6 or higher based upon the NCC Basic Skills placement test.

Eligibility Four

·         Passing grade in HMED D5008



Registration Procedure

  • Go to the Admissions Department, Room E106 to receive a Banner ID number if you have not received one previously.
  • If you meet one the eligibility categories listed above you may register for the programs in one of two ways;
    • A. Online through web registration if you have met these requirements through course work or taking Accuplacer at NCC
    • B. You will not be able to register online if you are requesting an exemption based on SAT or ACT scores or completed course work from another college or university. These exemptions must first be approved by a member of the Division of Nursing and Allied Health before you can register for the programs. See other side for guidelines.


Health Care Assessment Testing Procedures:

  • If you do not have a previous academic record or an NCC placement test score, please sign up using the following procedure.
    • 1. To sign up for a Health Care Assessment test, you must first receive your NCC Student ID (or Banner) number. 
    • 2. Go to Admissions, Room E106 OR click here for In-Take Form to receive a Banner ID 


College Credit Exemption

  • Students who have attended other colleges or universities may be exempt from the Health Care Assessment test. 

  • If you have taken previous college-level English and/or math credits and received a grade of C (2.0) or better, you are exempt from taking the Health Care Assessment test in that specific area.  Please submit a copy of your college transcript to a member of the Nursing and Allied Health Division, Rm. 113 in the Center for Science, Health and Wellness for evaluation.

  • If you have earned an associate's degree from an accredited U.S. college or university, NCC assumes that you have the English skills to do college-level work.  You are therefore exempt from taking the English portion of the Health Care Assessment test.  Please submit a copy of your college transcript to a member of the Nursing and Allied Health Division, Rm. 113 in the Center for Science, Health and Wellness for evaluation.

  • If you have earned a bachelor's degree or higher from an accredited U.S. college or university, NCC assumes that you have the English and math skills necessary for college-level work.  In most cases, you are therefore exempt from taking the Health Care Assessment test.  Please submit a copy of your college transcript to a member of the Nursing and Allied Health Division, Rm. 113 in the Center for Science, Health and Wellness for evaluation.



SAT and ACT Exemption

  • Students who have taken the College Board SAT or ACT exam may be exempt from the English and/or math sections of the Health Care Assessment test.

  • If your SAT Critical Reading score or SAT Writing score is 450 or higher or your ACT English score is 21 or higher or your composite ACT English and Reading score is 47 or higher: You are exempt from taking the English portion of the Health Care Assessment. Please submit a copy of your SAT or ACT scores to a member of the Nursing and Allied Health Division, Rm. 113 in the Center for Science, Health and Wellness for evaluation.

  • If your SAT Math score is 450 or higher your ACT math score is 18 or higher, you are exempt from taking the math portion of the Health Care Assessment test. Please submit a copy of your SAT or ACT scores to a member of the Nursing and Allied Health Division, Rm. 113 in the Center for Science, Health and Wellness for evaluation.


PLEASE NOTE: Retesting for the Health Care Assessment Test will only be granted with prior approval. Please contact Ilene Boyar, Testing Center Specialist at 203-857-3319 for retesting inquiries only.
