If you need a copy of the documents you have submitted to our office, please understand that the documents should be officially and lawfully obtained from the professional who issued and signed the report. When you receive a report from educational, medical or psychiatric professionals you should request two signed copies. One copy is for the organization that requires the documentation (the college) and the other copy is for your records.
A student request for a copy of their disability documetation will be honored only one time under exceptional circumstances. The student must meet with the Coordinator of Student Disability Services to make their request. Please use this Document Transfer request form.
If you would like a professional or organization to send medical, psychological, or educational documents to Student Disability Services in order to document your disabilities, you can use this Records Release request.

organization that requires the documentation (the college) and the other
copy is for your records. It is likely that you will need your
disability documentation for other organizations. The College is
required to retain these documents for 5 years from date of graduation
or last semester of enrollment in accordance with the State of CT
Records Retention Schedule S5. After 5 years the College is
permitted to shred the documents and dispose of them. I understand
that a student request for a copy of the documents regarding their
disability will be considered under very exceptional circumstances
one time only.
