If you would like a professional or organization to send medical, psychological, or educational documents to the Disability Services Office in order to document your disabilities, you can use this Records Release request.
If you need the Disability Services Office to send to another college or organization copies of the documents you have submitted to our office, please use this Document Transfer request.
