Disability Services Application Procedure
- Provide the appropriate documentation of
your disability, which should:
a. be from the professional who made the diagnosis (on letterhead, with signature, and license information);
b. be recent (preferably no more than three years old, one year for medical or psychiatric disabilities);
c. specifically name the diagnosed disability;
d. identify diagnostic testing mechanisms, procedures, and results [for recent high school graduates this means the Psychological Evaluation or Report and the Educational Evaluation usually including tests, such as, the Wechsler Intelligence Scales-(WAIS/WISC) and the Woodcock-Johnson Test of Achievement]
e. contain a narrative linking the testing results to the effect of the disability on learning and functioning in an educational environment;
f. and if relevant, identify any recommended accommodations.
- Obtain the forms in steps 3 & 4 from this
website or from the Disability Services Office (Room
209i on the West Campus or the bulletin board).
- Complete the
Intake Form.
- Complete the
Informed Consent Form.
- Deliver all medical, psychological or educational documentation,
the Intake Form and Informed Consent Form in a sealed envelope to the Disability Services Coordinator,
Room 209i on the West Campus.
- Schedule
an appointment for a meeting to
determine the appropriate services (see Appointment Sheet on the bulletin
board).
- If it is determined that you will need classroom or testing accommodations, follow the Accommodations Procedure.
Note: The demand for appointments becomes very high as the semester approaches. New students should contact the Disability Services Coordinator at least six weeks prior to the beginning of the semester. Continuing students (those who have provided documentation and previously met with the Disability Services Coordinator to discuss accommodations) should contact the Coordinator at/east three weeks prior to the beginning of the semester. Appointments will be scheduled on a first-come, first-served basis.
