Accommodation Request Procedure
-
Complete the Application Procedure.
- After registering for classes, obtain
the Accommodation Request Form from this website or from the
Disability Services Office bulletin board (Room 209i on the West Campus).
- Fill it out completely, listing only
those courses for which you would need accommodations.
- Bring the completed form to the the
Disability Services Office or send it via email to the Disability
Services Coordinator.
- Then make an appointment to pick up your
letters with the Disability Services Coordinator.
- Based on eligibility, you will be given
a letter to give to the instructor in each course for which you are seeking accommodations.
- Give the form to
each instructor as early in the
semester as possible, preferably on the first day of class.
- We rely on you to initiate this process each semester, when you want accommodations. In subsequent semesters, after registering for classes, obtain an Accommodation Request Form, and follow the same procedures.
Note: The demand for appointments with the Disability Services Coordinator becomes very high as the semester approaches. New students should contact the Counselor at least six weeks prior to the beginning of the semester. Continuing students (those who have provided documentation and previously met with the Counselor to discuss accommodation) should contact the Counselor at least three weeks prior to the beginning of the semester. Appointments will be scheduled on a first-come, first-served basis.
